Practice Information

Services

The Practice provides dental services including Crown & Bridge Work, Cosmetic Dentistry, Fillings, Dental Implants, Teeth Whitening, Preventative Care, Periodontal Care, Oral Surgery, and Invisalign. The Practice primarily operates on a fee‑for‑service model. The only in‑network insurance accepted is Delta Dental Premier. The Practice also offers an in‑house dental discount plan.

Appointments, Fees & Payment

• Scheduling: Appointments may be scheduled by phone at 480-991-0233 or through our online booking system. Online bookings are subject to confirmation by the Practice.
• Payment: Payment is due at the time services are rendered unless other arrangements have been agreed in writing. We accept major credit cards, cash, and checks.
• Estimates: Any fee estimates provided are estimates only and may change based on clinical findings at the time of treatment.
• Returned Checks: A returned check fee of $35 will apply for any check returned for insufficient funds or other reasons.

Cancellation & No‑Show Policy

• We request at least 24 hours' notice for appointment cancellations.
• Cancellations made less than 24 hours before the scheduled appointment or failure to appear for a scheduled appointment (no‑show) may result in a $50 fee. Repeat late cancellations or no‑shows may affect future scheduling privileges.

Insurance & Reimbursement

• The Practice is in‑network with Delta Dental Premier only.
• For patients with other insurance, we will provide the documentation necessary for you to submit claims to your insurer. Out‑of‑network reimbursement is determined by your insurance plan and is not guaranteed by the Practice. Any balance remaining after insurance reimbursement is the patient's responsibility.

In‑House Discount Plan

The Practice offers an in‑house dental discount plan. Details of the discount plan, including applicable services and discount levels, will be provided upon request. Membership in the discount plan does not constitute insurance.
